Air West Airlines
On September 2, 1978 an Air West de Havilland DHC-6 Twin Otter crashed into a marine on the waterfront in Vancouver, BC. The investigative work conducted by Dr. R.J. Waldron led to the determination of a flap rod failure having caused the accident.
